TRANSITIONS PARTIAL HOSPITALIZATION
Partial Hospitalization Program in Napa and VallejoTransitions is an accredited partial hospitalization program offering intensive outpatient mental health treatment to adults 18 years and older. It allows individuals to remain in their own community and living situation. The program is highly structured and goal oriented, and is designed to increase individuals' levels of functioning through various treatment modalities.
